EVOLVE YOGA LOUNGE PARTICIPATION AGREEMENT & RELEASE OF LIABILITY By electronically signing this agreement, I acknowledge and agree to the following terms for myself or, when applicable, a minor participant under my legal care. 1. Consent to Participate I voluntarily agree to participate in any yoga, pilates, fitness, meditation, sound healing, wellness services, workshops, training programs, and special events (collectively, the "Activities") offered by Evolve Yoga Lounge, LLC ("Evolve"). Activities may take place at the studio, off-site, outdoors, virtually, or through digital content. 2. Health & Safety I understand that participation in physical and wellness Activities involves inherent risks, including injury, illness, or aggravation of existing medical conditions. I acknowledge that: • I am responsible for determining my ability to participate safely and consulting a healthcare professional when appropriate. • I will notify my instructor of relevant injuries, health concerns, or physical limitations. • I will modify or discontinue participation whenever necessary. • Evolve may limit or refuse participation for safety reasons. 3. Assumption of Risk & Release of Liability I voluntarily accept the risks associated with participation in Evolve's Activities. TO THE FULLEST EXTENT PERMITTED BY TEXAS LAW, I RELEASE AND HOLD HARMLESS EVOLVE YOGA LOUNGE, LLC, ITS OWNERS, EMPLOYEES, INSTRUCTORS, CONTRACTORS, GUEST PRESENTERS, AND AFFILIATES FROM CLAIMS ARISING FROM MY PARTICIPATION, INCLUDING CLAIMS ARISING FROM THEIR ORDINARY NEGLIGENCE. This release includes claims involving personal injury, illness, emotional distress, property damage, or death, to the extent permitted by law. I understand that I am giving up certain legal rights, including the right to bring claims covered by this agreement. 4. Photography & Media I understand that Evolve may photograph or record classes and events for educational, marketing, or promotional purposes. I may separately authorize Evolve to use my image or likeness, or that of a minor for whom I am the parent or legal guardian, in photographs, videos, websites, social media, and promotional materials. 5. Parent or Legal Guardian Consent for Minors If I am electronically signing on behalf of a participant under 18 years of age, I confirm that: • I am the minor's parent or legal guardian and legally authorized to provide consent. • I authorize the minor to participate in Evolve's Activities. • I understand and acknowledge the risks associated with the minor's participation. • I am responsible for providing accurate information regarding relevant medical conditions, allergies, injuries, or limitations. • I authorize Evolve to contact emergency services when reasonably necessary. I agree to the applicable terms of this agreement on my own behalf and understand that my electronic signature does not necessarily waive independent legal claims belonging to the minor. 6. Governing Law This agreement is governed by the laws of the State of Texas. Any legal proceedings relating to this agreement shall be brought in a court of competent jurisdiction in Denton County, Texas, where legally permitted. If any provision is found unenforceable, the remaining provisions shall remain in effect. 7. Electronic Signature & Acknowledgment BY ELECTRONICALLY SIGNING THIS AGREEMENT, I CONFIRM THAT: • I have read, understand, and voluntarily agree to these terms. • I acknowledge the risks associated with participation. • I understand that this agreement includes a release of liability and may limit my legal rights. • I am at least 18 years old or am the parent or legal guardian signing on behalf of a minor. • I consent to the use of my electronic signature, which I intend to have the same legal effect as my handwritten signature. This agreement applies to my current and future participation in Evolve's Activities unless replaced or modified by a subsequent agreement. Evolve Yoga Lounge, LLC

Founding Membership Agreement, Parental Consent & Liability Waiver Evolve Yoga Lounge · Empower Girls Yoga Circle Welcome to Evolve Yoga Lounge. We're honored to give your daughter a safe, supportive place to build confidence, mindfulness, strength, and joy through movement. This Agreement is between Evolve Yoga Lounge, LLC ("Evolve") and the parent or legal guardian purchasing this membership ("Guardian"), on behalf of the child enrolling ("Minor"). By purchasing this membership, you agree to all of the terms below — including the release of liability and waiver of claims on behalf of yourself and your child. 1. Your Membership — $200/month Your membership includes: * 12 class credits each month |shared between 3 Siblings * Credits roll over while your membership is active and in good standing * Automatic $200 monthly draft * Membership covers only the enrolled siblings and can't be transferred 2. Billing Dues are drafted automatically each month from the payment method on file. * A 2.9% processing fee applies to all transactions. * Declined or returned payments are charged a $25 fee. * Accounts more than 7 days past due may be suspended until payment clears. 3. Cancelling Cancel any time by emailing info@evolveyogalounge.com at least 30 days before your next billing date. Your membership stays active through the period you've paid for. Unused credits expire when you cancel and have no cash value. Cancelling means giving up your founding rate. 4. All Sales Are Final All purchases at Evolve Yoga Lounge are final — memberships, class packages, workshops, camps, events, and retail. We don't issue refunds for any reason, including missed classes, schedule conflicts, illness, injury, moving, personal circumstances, or dissatisfaction. Evolve may issue studio credit case by case, at our discretion. Studio credit has no cash value, can't be transferred, and may expire. 5. Parent & Guardian Consent You confirm that you are the Minor's parent or legal guardian, that you have legal authority to sign this Agreement, and that you consent to your children taking part in yoga classes, movement activities, wellness programs, workshops, camps, and related activities at Evolve. Participation is voluntary. 6. Health & Emergency Care You confirm your children are physically able to participate, and you agree to tell us about any medical conditions, allergies, injuries, developmental concerns, or anything else that could affect her participation. If we can't reach you in an emergency, you authorize Evolve staff to get emergency medical care for your child or children, and you accept financial responsibility for that care. 7. Risks of Participation Yoga, movement, stretching, balance work, mindfulness exercises, games, and wellness programming carry real risks — including falls, muscle strains, sprains, physical exertion, worsening of existing medical conditions, and serious bodily injury. You knowingly and voluntarily accept all of these risks, known and unknown, on your children's behalf. 8. RELEASE OF LIABILITY, INCLUDING NEGLIGENCE You, The Guardian, on behalf of themselves and the Minors, releases, waives, discharges, and agrees not to sue Evolve Yoga Lounge, LLC, or its owners, officers, employees, instructors, independent contractors, volunteers, agents, and representatives for any claim arising out of the Minor's participation. THIS RELEASE EXPRESSLY INCLUDES CLAIMS ARISING FROM THE NEGLIGENCE OF EVOLVE YOGA LOUNGE, ITS OWNERS, EMPLOYEES, CONTRACTORS, INSTRUCTORS, OR REPRESENTATIVES. It covers claims for personal injury, illness, disability, death, and property damage. 9. Indemnification Guardian agrees to defend, indemnify, and hold harmless Evolve Yoga Lounge, LLC, its owners, employees, contractors, instructors, volunteers, and representatives against any claims, liabilities, damages, expenses, or attorney's fees arising from the Minor's participation, the Minor's actions or conduct, or any claim brought by or on behalf of the Minor. 10. Photos & Video We sometimes photograph or record classes and studio activities for teaching, training, marketing, promotion, and social media. By enrolling, you give Evolve permission to use your child's image, likeness, voice, or appearance in photos and video without payment. If you'd rather your children not appear in promotional media, give written notice to studio management before she participates. We'll make reasonable efforts to honor that, though incidental background appearances can happen. All media remains the property of Evolve Yoga Lounge. 11. Studio Expectations We ask parents and children alike to help keep this a positive, respectful, supportive community. Evolve may suspend or end a membership without refund for behavior that is disruptive, unsafe, abusive, or out of step with our policies and values. 12. Personal Property Evolve isn't responsible for lost, stolen, or damaged belongings. 13. Texas Law This Agreement is governed by the laws of the State of Texas. Any dispute will be resolved exclusively in the state or federal courts of Denton County, Texas. If any part of this Agreement is found unenforceable, the rest stays in effect. Parent / Legal Guardian Acknowledgment Purchasing this membership constitutes your acceptance of this entire Agreement, with the same force as a signed document, on behalf of yourself and your children. By purchasing this membership, you acknowledge that: * You have read and understand this Agreement. * You voluntarily agree to all of it, for yourself and your children. * You understand you are giving up substantial legal rights, including claims arising from the negligence of Evolve Yoga Lounge and its representatives. * You understand all dues and purchases are final under our Final Sale Policy.
